I would like to change from a Backer's Edition Physical Vita Cart to a Physical PS4. Is this still possible or am I too late?

It's never late to change your way of living.
Anyway, you probably (actually, certainly) received an e-mail from Fangamer after the first time you answered your survey. Open that e-mail and click on the link there, you'll be redirected to a page where you can change your survey answers (and change which version you want), upgrade your tier and other things.
