Would love to see a Nightmare-level difficulty mode designed to challenge the player while using their best and most maxed out character build on a NG+ run from game start to finish.
The regular Nightmare mode does a good job providing challenge for the player on a new run but there isn't anything comparable for NG+ runs and at least for me I spend most my time playing NG+.
A new difficulty mode that makes the whole NG+ game as challenging as the regular Nightmare endgame would be an ideal solution.
This would also provide a new level of challenge for Boss Rush Mode where the earlier bosses tend to go down within a few seconds and provide little if any challenge.

Thing is, it's hard to predict what an NG+ run looks like. After a playthrough you might have everything or very little stuff. I don't quite know how they'd adjust the difficulty for every conceivable setup.

I actually don't quite understand the question, there is already a Nightmare game available for NG+ so I'm guessing you mean a Nightmare+ for NG+ which is quite a good idea, but I only played it on Normal and it makes me frustrating many times already, I want to say no but I would more like to give it a yes to any new interesting feature

There isn't really much you can do to make the game more difficult outside of just buffing enemy stats and placements to absurdity. The reason why NG+ is so easy is because you have access to all of your combat tools from the get go, power doesn't really matter as much. It doesn't matter if there's 50 enemies in a room if I never need to go near them because Tis Rozain/recycle hat/timestop+glasses lets me snipe them from across the screen at zero risk and cost.

For example it should take you around the same time to take down Vepar as it takes to defeat the final few bosses. The same concept goes for regular enemies too. There isn't really much you can do to make the game more difficult outside of just buffing enemy stats and placements to absurdity. The reason why NG+ is so easy is because you have access to all of your combat tools from the get go, power doesn't really matter as much. I think what you're describing applies if your play-style is one where you take little risk if any. If you're sniping enemies from a safe distance (nothing wrong with that, to each their own) then you'd be correct that enemy stats are kind of irrelevant. But if you have a more aggressive play-style and like to get in close to the enemy, their stats do matter. This is where a Nightmare mode difficulty with equally strong and tough enemies and bosses comes in.
