I got 21, poor Iga's included... I didn't miss anything, did i?
Also, are they limited, throughout difficulties/runs? I got 8-bit one in Normal, yet while beating Overlord no-damage again in Hard, no new/additional medal was ever acquired.

Despite key items being able to list quantity, it is impossible for any of them to have a quantity over 1. So if you start NG+ game, any recipe / hair apparent etc items, when acquired again, will not add to the inventory count. This also applies to boss medals.
There are 21 boss medals in total, including IGA. the Bonus boss OD does have a medal. The three extra bosses, Master Carpenter, Millionaire's Bane and Revenant, do not have medals. Dominique, Bael and 8-bit Nightmare's medals are the only non-missable medals, as you can attempt these fights as many times as you like during a single playthrough.
