Occurrence :
Observed once.
In the room with the single troll axe thrower and the breakable wall on the right hiding a HP Max-up, I somehow got the game into a state where even after breaking the wall and collecting the HP Max-up I could re-enter the room and the broken wall would revert to its unbroken state with a new HP Max-up to collect. I was mostly trying to farm the axe thrower's shard but I ended up gathering that one HP upgrade 3 times before I thought about reloading from the nearby save room. Upon reloading, the wall stayed broken and worked as normal.
Severity:
Infinite. HP. Max-ups. I don't think that kind of severity would need to be explained.
Reproducible:
Unknown; I've only encountered it once and could not get it to happen again. It might have something to do with me playing through to that point without reloading from a save at all, but I'm not sure.
I don't expect anyone to believe this, and it didn't occur to me to screen record the glitch as it was happening (my PC is pretty old anyway), but yeah.
